1. Attendance and participation: I understand that this programme requires regular attendance at scheduled classes, tutorials, workshops and assessment appointments. I must notify Moorland Education promptly if I cannot attend and provide evidence where reasonably requested.
2. Online and independent study: I understand that I am required to complete online learning, independent study, reading, activities, assignments and other tasks by the deadlines set by my tutor or assessor.
3. Workplace assessment: I understand that this is a competence and work-based programme. Skills-based outcomes may require direct observation of my performance in a real work environment and other valid workplace evidence. I agree to cooperate with reasonable assessment arrangements and to help Moorland Education liaise with my employer or workplace where necessary.
4. Learning diary / reflective log: I agree to maintain any required learning diary, reflective log, workplace record, portfolio or evidence tracker accurately and submit it when requested. Entries and evidence must be genuine and must protect the confidentiality of people who use services.
5. Assessment evidence: I understand that evidence may include observation, professional discussion, written or oral questioning, learner statements, products of work, assignments, projects, case studies, expert testimony and other methods permitted by the qualification requirements.
6. Authenticity and academic integrity: All work submitted must be my own unless collaboration is expressly permitted. Plagiarism, falsification of workplace evidence, forged signatures, fabricated diary entries, impersonation or other academic misconduct may lead to disciplinary action and may be reported to the awarding organisation where required.
7. Confidentiality and safeguarding: I will not include unnecessary personal or confidential information about service users, colleagues or employers in coursework. I will follow workplace safeguarding, confidentiality, data protection and information-handling requirements.
8. Employer cooperation: I give permission for Moorland Education to contact my employer, manager, supervisor or referee for legitimate enrolment, assessment, quality assurance and qualification purposes, including confirmation of my role and workplace evidence.
9. Changes in employment: I must tell Moorland Education promptly if I leave my employer, change role, move workplace, take extended absence or otherwise lose access to a suitable work environment, as this may affect my ability to complete competence-based assessment.
10. Conduct: I will behave professionally and respectfully towards learners, staff, assessors, employers and service users and comply with Moorland Education's relevant learner policies and procedures.
11. Progress and deadlines: I am responsible for engaging with feedback, attending progress reviews, completing required work and responding to reasonable communications from Moorland Education.
12. Suspension or termination: I understand that serious misconduct, falsification of evidence, persistent unauthorised absence, repeated failure to participate in online or classroom learning, failure to complete required workplace assessment or diary/portfolio evidence, or continued breach of programme requirements may result in suspension or termination from the course.
13. Award of qualification: Enrolment does not guarantee achievement. The qualification can only be awarded after I have successfully met all required learning outcomes, assessment criteria and quality assurance requirements.
14. Information accuracy: I confirm that the information supplied in this form is true and complete to the best of my knowledge. I understand that materially false or misleading information may affect my enrolment or registration.
